Clinically, hemoglobin typically begins to rise within 1–2 weeks post-infusion, but a meaningful increase is often seen by 3–4 weeks, assuming ongoing losses are controlled and there’s no other cause of anemia.

Symptom improvement (fatigue, shortness of breath, cognitive fog) can occur within a few days to 1–2 weeks, though this varies from patient to patient

• An iron infusion is done using Monoferric Iron and given through an IV in your arm

• Indicated when oral iron is ineffective, poorly tolerated, or too slow

• Iron is taken up by macrophages and hepatocytes, then gradually transferred to transferrin.

• Iron stores (ferritin) increase first, followed by incorporation into erythropoiesis.

• After an iron infusion, your body starts absorbing the iron and storing it, mainly in the liver and immune cells. These stores act like a backup supply. Once your iron stores are filled up, your body begins using the iron to make new red blood cells.

Concerns include:

• Cost and limited coverage

• Risk of adverse reactions, though rare

• Regulatory limitations: It is difficult to meet approval standards to receive an infusion through Alberta Health. At this time, your ferritin not only has to be extremely low but your Hemoglobin must be less than 95

Not necessarily "serious," but it indicates:

• Significant iron deficiency not responding to or not suitable for oral therapy

• A need to address the underlying cause -is it heavy/long menses, haemorrhoids, undiagnosed Celiac (malabsorption) disease

Generally safe when proper protocols are followed. Serious adverse effects are rare:

• <1% risk of anaphylaxis when using Monoferric Iron

• Infusion reactions (e.g., flushing, nausea, back pain) occur in <5% and are usually mild and go away when the infusion is complete

Therapeutic effect: May last months to years, depending on iron loss and stores.

There’s no absolute threshold, but commonly considered when:

• Ferritin <20 ng/mL (absolute IDA)

• Ferritin 30–100 ng/mL with TSAT <20% (functional deficiency)

• If you do not fall within these ranges, it may still be worth doing a complimentary consult with our physician for consideration

Ferric derisomaltose (Monoferric): For doses of 500-1000mg: Approx 30 minutes to infuse and 15 minutes for a follow up mini Saline bag to infuse. The nurse is usually at your home for 1 hour. 

For doses of 1000mg+ the appointment is about 1.5 hours

Coverage varies by province and clinical setting:

• In Alberta, iron infusions are typically covered if administered in hospital or AHS outpatient clinics and if criteria are met.

• Private infusion clinics often require out-of-pocket payment or private insurance.

• Special Authorization may be required for provincial drug plan coverage in community pharmacies.

Mild Reactions- feeling flushed, chest or back pain, nausea. These symptoms are often anxiety driven that manifest as physical symptoms during the infusion. They are often relieved by knowing these are not uncommon symptoms and they go away once the infusion is complete

Severe anaphylactic reactions: low risk approx 0.1% of people

Iron Staining-low risk of the iron leaking out of the vein into surrounding tissue, creating a brown/grey colored stain of the skin.

Lowered blood pressure-mild drop can occur due to dilation of blood vessels

Delayed reaction-Occurs in 10-20% and starts 1-2 days after infusion and includes fatigue, aches and chills, low grade fever
